Consent Form

Registration Process for Westlake Preparatory Lutheran Academy:

1. Application for Enrollment Form must be completed and submitted with the appropriate
   Registration Fee - $250 (non-refundable unless the student is denied enrollment)
    

2. Recommendation Form must be completed.
   The individual completing the form should mail the recommendation directly to
   Westlake Preparatory Lutheran Academy.  Attn: Admissions Office
    

3. Submit copies of the items below to the school office:
   Birth Certificate (grades PreK-5 only)
   Standardized Test scores (grades 2-5 only)
   Report Card (most recent from your current school)
   Immunization Records (complete history)
   A copy of any Specialized Testing the applicant may have taken (PreK- 12th)
    

4. Schedule Admissions/Placement Testing
    

5. When steps 1-3 have been completed, the school will contact you for an interview..
